Where to find code that writes the Edit & Upload Videos forms to the database?

Hi Mike. I am finishing my custom hacks to make SIM Gallery work with JomSocial Groups on my site. On the Edit Video page, where is the code (file/path/line number) that writes this html form: <form id="edit-video-form" . Also, where is the query (file/path/line number) that writes this form's input to the database?

Then on the Videos album page, there is the "+ Video" button that triggers the popup to appear. Where is the code that writes this form in the popup: <form id="addvideo-form"
Finally, where is the query that writes the form input from this popup to the database? Thanks!
